As a Trusted Community Partner, we already have all your organizational information on file, so this brief application focuses squarely on what SMCF needs to facilitate a pass-through donation.

If, however, your organization lacks internet access, no stress! You can pick up a paper copy at our office, and we’ll upload your typed or handwritten responses for you.
